The following information is requested in order to have a booth at the Symposium: \nPayment request amount: Avera requests that interested vendors remit payment of $1000 to have a booth at the Symposium for the 28th and 29th of September\, 2017. \nAgenda:  Draft Agenda \nAvera McKennan Tax ID Number: 46-0224743 \nPayment:\nPlease click the Register button to register.  If able to pay by credit card simply fill out that information.  If you need to pay by check please click the register button and register.  Please make checks payable to Avera McKennan and send to – \nAttn: Marcia Smith\nAvera Cancer Institute\n1000 E. 23rd Street\, Ste.340\nSioux Falls\, SD 57105 \nAnticipated number of attendees: 175 – including MDs\, NPs\, CNPs\, social workers\, patient navigators\, and all others both inside healthcare and in the community who are interested in learning more about the current state and future prospects of cancer treatment and facilities. \nVendor Area: The vendor booths are setup in an area for viewing that is distinct from where the symposium education is occurring\, and each booth has their own small area that is distinct from other vendors around them. \nBreak for viewing: Breaks are listed on the agenda\, however if attendees at the symposium choose not to view some sessions offered they may also view vendor booths during the entirety of the program. \nIf you have any questions or concerns please contact the Administration Office at the Avera Cancer Institute\, 605-322-3276. Thank you for your interest in our Symposium\, and we hope to see you there. \nAccreditation\nCME:\nAvera is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. Application has been made for continuing medical education. \nCNE:\nAvera Health is an approved provider of continuing nursing education by the Alabama State Nurses Association\, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Application has been made for continuing nursing education. \nRespiratory Therapy:\nApplication will be made to American Association for Respiratory Care (AARC) for continuing education contact hours for respiratory therapists. \nPharmacy:\nApplication will be made to the South Dakota Board of Pharmacy for Pharmacist Continuing Education credits. \nLong Term Care Administrators:\nApplication has also been made for credit for nursing facility administrators\, social workers\, and counselors \nRecording Attendance\nProgram participants should bring their cellphone with them to the conference. Program check-in for this event will be taken via text message. \nDress in Layers\nWe recommend dressing layers to allow for personal adjustments to the room temperature.
